Hendrich Fall Risk Model
The Hendrich fall risk model (HSM, also Hendrich scale) is a nursing instrument designed by US nursing scientists, especially Ann Hendrich, to assess the risk of falling sickness of a patient or patient. Within the framework of the nursing history, six so-called items (factors) are assessed and assessed, which allow an assessment of the risk of falling: prehistory, depression, confusion / disorientation, altered excretion, dizziness and inappropriate mobility / weakness. In accordance with the risk assessment, fall prevention is included in nursing planning and adapted in the case of changes in status. Further possibilities to evaluate the risk of falling are the morse scale and STRATIFY, which deal specifically with the fall evaluation; other tools such as the care dependency scale record additional nursing.
